The Rural Hand Therapy Project: A mixed methods study in a regional health service
Rural occupations carry a high risk of hand injury. In rural locations, provision of hand injury rehabilitation often falls to generalist occupational therapists. However, these therapists' capacity to provide hand injury assessment and treatment is limited. The Rural Hand Therapy Project (RHTP) was developed to improve access to hand therapy for patients living in rural areas by supporting rural generalist occupational therapists to provide hand therapy. The aim of this study was to investigate the impact, acceptability and experience of the RHTP for patients and clinicians.

Memory benefits of daily-living-related contextual cueing for individuals with subjective cognitive decline and mild cognitive impairment
Objective We aimed to assess how daily-living-related contextual cueing (DLCC) affects memory performance in three groups: healthy older adults (HA), those with subjective cognitive decline (SCD), and mild cognitive impairment (MCI), while accounting for age and education. Methods After gathering demographic information, participants underwent neuropsychological assessments including the Montreal Cognitive Assessment (MoCA), Everyday Cognition Scale (ECog-12), Mini Mental State Examination (MMSE), Geriatric Depression Scale (GDS), and Trail Making Test (TMT) to establish baseline characteristics. Participants were categorized into HA ( N = 47), SCD ( N = 54), and MCI ( N = 43) groups based on MoCA and ECog-12 scores. Memory performance was evaluated through two components of the Contextual Memory Test (CMT): one with non-contextual cues and the other with daily-living-related contextual cues. Results Interaction effects between contextual cueing and group for immediate recall ( p < .001), delayed recall ( p < .001), and total recall ( p < .001) were found. All recall scores were lower in the MCI group than in the other two groups in the contextual cueing, not non-contextual. The post-hoc results revealed that scores on immediate recall, delayed recall, and total recall were lower in the MCI group than in the other two groups in the contextual cueing condition but not in the non-contextual cueing one. Conclusions Daily-living-related contextual cueing benefited HA, SCD, and younger-adult MCI groups more than older-adult MCI group, particularly enhancing delayed and total memory performance.

A Comparative Study on Proprioceptive Neuromuscular Facilitation Versus Motor Relearning Programme on Improving Functional Recovery in Stroke Patients.
Stroke is one of the leading causes of disability worldwide, significantly impairing motor function and daily living activities. Rehabilitation plays a crucial role in the recovery process, with various therapeutic approaches aimed at restoring motor function and improving quality of life. Two prominent rehabilitation interventions, Proprioceptive Neuromuscular Facilitation (PNF) and the Motor Relearning Programme (MRP), have demonstrated effectiveness in improving stroke recovery. However, the comparative efficacy of these approaches remains underexplored.This study aims to compare the effectiveness of PNF and MRP in improving functional recovery in stroke patients. Specifically, the study evaluates the impact of these interventions on motor function, balance, and activities of daily living.

Occupational therapists’ perceptions of the relationship between gravitational insecurity, emotion regulation and functioning in adult mental health settings: an interpretative phenomenological analysis
Gravitational insecurity (GI) is characterised by a significant fear of movement that is disproportionate to the level of threat and any deficiencies in postural control. It has been suggested to lead to increased anxiety and barriers within everyday activities. Limited insight is available into how this difficulty may present in adults, specifically those with mental health conditions. This study aims to consider occupational therapists’ perceptions on how GI may alter emotional regulation and functioning in this population.

Occupational consciousness: Theorising to dismantle systemic racism and dehumanisation
This paper is based on the 2023 Dr. Ruth Zemke Lecture in Occupational Science. It unpacks how the concept, occupational consciousness, can help us theorise around dismantling systemic racism and dehumanisation. I first argue that systemic racism and dehumanisation are intricately linked. Essentially, racism instigates or stirs up the genocidal features of dehumanisation. I then ask the question: why is it that racism, which is ultimately evil, so intractable? What follows is my theoretical explanation to the intractability and perceived normality of this evil, by sketching how human co-option into systemic racism occurs at four levels: geopolitical; national; professional (institutional); and personal, through human occupation that intersects everydayness with extractivist capitalism. To illustrate how theorising can help dismantle systemic racism and dehumanisation, I offer three examples of scholarly praxis in raising occupational consciousness. The adoption of occupational consciousness beyond occupational science and occupational therapy demonstrates the potential reach of theorising human occupation. This is important to consider not only as we envision the future of occupational science, but also as we critically appraise the discipline’s origins and evolution to date.

Why Occupational Therapy?: A Qualitative Analysis of Incoming Students’ Reasons for Entering the Profession
Insight into students’ views and understandings of what occupational therapy (OT) practitioners do and what a career will be like can shed light into how the profession is socially perceived. Additionally, this knowledge can be useful for recruitment, retention, and curriculum design, especially for addressing misunderstandings and inaccurate perceptions. Therefore, the objective of this study was to examine the narratives of incoming OT students when asked why they chose to enter the profession of OT. Descriptive statistics were used to analyze sociodemographic data, and then thematic analysis was used to analyze qualitative data about why participants wanted to become occupational therapists. The qualitative analysis of narratives from 67 incoming OT graduate students resulted in five themes: (1) understandings of disability; (2) caring/helping; (3) a healthcare profession – but different; (4) perceptions of day-to-day practice; and (5) past exposure to OT. The findings from this study identifies that many people choose a career in OT specifically because of its unique blending of artistry and science as a health profession. However, many of these individuals enter the profession with misperceptions of disability and barriers to occupational participation that clients/patients face. Faculty can use the findings of this study to recognize students enter the academic world with many misperceptions, including of living life as a person with disability. Additionally, the findings may identify approaches for leveraging what makes OT distinct in the eyes and minds of students exploring and considering careers in healthcare.

Occupational Therapy Embedded Within Pulmonary Rehabilitation: A Process Evaluation
Exploration of the impact and contributions of occupational therapy within multidisciplinary pulmonary rehabilitation (PR) is necessary to strengthen the clinical evidence-base. The objective of this study was to describe occupational therapy embedded within a community-based PR program and explore mechanisms of impact. A process evaluation of occupational therapy practice embedded within an 8-week multidisciplinary PR was conducted. Data sources included administrative and medical record data, intervention checklists, and semi-structured interviews incorporating the Canadian Occupational Performance Measure and Modified Borg Dyspnea Scale. Nine participants completed the PR program. The home visit and personalized, one-to-one, occupation-centered interventions were identified as key mechanisms of impact. Goal-directed training occurred within the home but not in community clinic environments. The repetitive, occupation-based training was important for learning and everyday application and promoted application of multidisciplinary learning. This evaluation highlighted that individualized occupational therapy assisted PR patients to apply knowledge, from occupational therapy and other disciplines, to real-life challenges.

Physical, Cognitive, and Psychosocial Intervention in Older Adults with Subjective Cognitive Decline: Clinical Protocol for a Randomized Controlled Trial
People living with subjective cognitive decline (SCD) have a high risk of future cognitive decline and progressing to dementia. Lifestyle risk factors that can be changed have been recognized as significantly contributing to cognitive decline. Therefore, implementing strategies to address these factors offers a considerable chance to minimize the cognitive decline and incidence of dementia among individuals. The most recent guidelines to prevent cognitive decline is multicomponent interventions. This trial shall aim to help develop practical strategies for enhancing brain and cognitive health in Indian older adults with SCD.
Perspectives of School-Based Occupational Therapists
In the United States, schools are the second largest employer of occupational therapists (AOTA, 2020a). While research on school-based topics exist, to date there is no comprehensive study on school-based occupational therapy (OT) practice. The purpose of this study was to describe current implementation of the school-based OT process and better understand perspectives of school-based occupational therapists.
